What is Nevada Check Up?
Nevada Check Up is the State of Nevada's Children's Health Insurance Program. It provides low-cost, comprehensive health care coverage to working families with uninsured children 0 through 18 years 11 months of age who are not covered by private insurance or Medicaid.
How do we apply?
The process is easy!
You can download the application from our website or access the online application for submission from our website . You can also call toll free 1-877-543-7669 and request an application to be mailed to your home adddress.
Make sure to include the following:
- Proof of all household income must be included with your application.
- Copy of the child(s) birth certificate, social security card or legal resident card
- Any abnormalities in loss of health insurance due to circustances beyond your control must be explained (move, loss of job)
- For wages or salary from jobs, enclose copies of your 2 most recent pay stubs (proof of one month of income).
- If you are self-employed, enclose your most recent tax return (IRS Form 1040) along with Schedules C, E, or F for all businesses you own.
- If you receive Social Security, Child Support, Disability, or any other form of unearned income, enclose a copy of the award letter, court order, a bank statement showing direct deposits, a copy of the check, or any other proof you may have that shows the income source, the amount, and how often you receive the money.
Your Nevada Check Up application will be reviewed within a minimum of 45 days and sometimes longer depending upon applications submitted during tough economic times. You will be notified by letter whether you are approved or denied for coverage and you can call to request information on the processing of your application to 877-543-7669.
