The head-of-household completes the application and mails it to the Nevada Check Up office. Make sure to include the following:

  • Proof of all household income must be included with your application.
  • For wages or salary from jobs, enclose copies of your 2 most recent pay stubs.
  • If you are self-employed, enclose your most recent tax return (IRS Form 1040) along with Schedules C, E, or F for all businesses you own.
  • If you receive Social Security, Child Support, Disability, or any other form of unearned income, enclose a copy of the award letter, court order, a bank statement showing direct deposits, a copy of the check, or any other proof you may have that shows the income source, the amount, and how often you receive the money.
